Host Home Provider

Residential Habitation- Individual Residential Services and Supports (Host Home Providers and Family Caregivers)

Connect. Grow. Thrive. provides Individual Residential Services and Supports (IRSS) for up to three people (3rd room has to be approved by the third room waiver) receiving services in a family and host home setting. A variety of living arrangements are established to meet the unique needs for support, guidance, and habitation of each person under our care. We are committed to helping people work towards their dreams and goals while offering high quality services.   

Each person receiving services lives and participates as independently as possible in their community, and receives IRSS to acquire, retain, and/or build on the necessary skills to live as independently as possible. Goals are determined by the client, or client’s guardian, to find the best matched residential setting to facilitate growth, increase independence, and aid in skill acquisition. IRSS are planned and coordinated through each person’s Service Plan (SP), which includes details on the scope, frequency, and duration of the services. These services are determined by the person’s strengths and needs, which are identified in their SP.

Connect. Grow. Thrive. ensures the health, safety, and welfare of each person they support. IRSS includes but is not limited to: 

  • On-going supervision as outlined in each person’s SP 

  • Education, training, modeling, cues and supports for all activities and areas of daily living 

  • The full extent of self advocacy 

  • Training to recognize, prevent and report incidents of mistreatment, abuse, neglect, and exploitation

  • Experience, exposure, and education on the ADA act

  • Communication skills 

  • Community navigation skills

  • Household skills

  • Emergency prevention, response, and preparedness training

  • Community integration and participation in a wide array of experiences 

  • Promotion of natural supports, social roles, and relationships 

  • Transportation to preferred activities

  • Education and supports with money management

  • Health and medication management 

  • Behavioral interventions and therapies by licensed therapists and specialists
