Connect. Grow. Thrive. helps participants access a wide array of experiences and education in their community life. Types of experiences are including but not limited to typical experiences of people in the general population, such as community leisure, recreation, education, training, and volunteer opportunities. Individual Residential Services and Supports (IRSS) is not limited to socialization and adaptive skills; personnel will accompany and support each participant to retain, improve, or acquire skills. Connect. Grow. Thrive. offers individualized settings as the learning environment, to provide one-on-one services as identified in each person’s Service Plan (SP).
Connect. Grow. Thrive. will provide a vast array of opportunities to facilitate and build relationships that provide exemplary support in the community and Colorado’s natural areas supporting the skills and abilities of each individual served. Utilizing the community and local environments as an atmosphere to learn and provide services that support identified goals in our participants’ Service Plans. Such activities will be conducted in various settings where participants can interact with non-disabled individuals other than those individuals who are providing services to the participant.
All individuals in our day program services will get a chance to help in planning the activities calendar each month.
We are looking to hire individuals to facilitate the Community Connection Program in Larimer and Weld counties.
