Connect. Grow. Thrive. strives to connect children and adults with intellectual and developmental disabilities* to host home providers who help our clients thrive in their home and community environment. We work closely with each client to ensure that they are able to grow in their respective home environment, as well as with our host home providers, making sure that they are given the best support and training to be able to provide the best care. 

Connect. Grow. Thrive. uses the People-Centered Model to better the lives of individuals with their daily and long term living environment. Through personalized Service Plans that focus on the individuals goals of each client, we put our clients at the center of our practice. 

We also offer a variety of other services. Our clients can take part in the Community Connection Program, to be built around the wants and needs of individuals in our care. 

Connect. Grow. Thrive. has a network of trained respite providers, who provide relief care for our host home providers and family members who take care of developmentally or intellectually disabled individuals. 

Finally, we provide non-medical transportation to and from daily activities for the Community Connection Program. 

*We require children with developmental and intellectual disabilities to be on the CHRP Waiver and adults with developmental and intellectual disabilities must be on the DD Waiver.
